Making It Happen encourages people to acquire the necessary skills and confidence for lifelong learning and growth.
This outcome-based program is designed to assist adults ages 18 – 28 to acquire the skills necessary to transition from school to life in the community.
Participants will work with their transition coach, family, friends, and community connections to develop skills to achieve goals and define their independence. Individualized supports are developed, based on intensive goal-setting and planning designed to build connections to natural support networks.
The Making It Happen Program operates from 101 Stewart Street in Chatham.
Planning / Supports Offered
- Your transition coach will assist you to be creative, to dream, and to identify your goals
- We will help to remove barriers, and look for other choices in order that you can experience successful options
- Planning/supports may include areas such as: building self-confidence and self-esteem, advocacy, speaking out for oneself, skill development, building relationships, partnerships, and natural supports
- Your transition coach can also assist with accommodations, budgeting, recreational activities, medical appointments, employment, and educational options
